Notes for completing the application:

• For the methods and deadlines for submitting the application, refer to the office of interest;

• Fill in the form in the editable parts (the boxes) , save the application, print it, sign it at the bottom with handwritten signature (the signature must be affixed in full and legibly), and send it as an attachment only in PDF format - name the file: " SURNAME Name DOM AMMIS PRAT FORENSE " alternatively print the form and fill it in by hand;

• Enter the dates in the format 01.01.2021 or 01/01/2021;

• It is mandatory to indicate telephone numbers and e-mail address, always also enter an uncertified mailing address to speed up communications ;

• Enter only the compulsory fundamental exams of the required subjects ;

• Report the name of the exam exactly as in the degree certificate and enter the grade, as in the example, the average of the marks is the responsibility of the Office ;

Example of Civil Procedure matter :

if compiled by hand, also write the grade in letters

• For courses of study with a Specialized Degree, enter the fundamental subjects of both the three-year and two-year periods (as if it were a single course of study) and for the degree grade only that of the Specialized Degree.
